Overview:
Our client, a leading hospital system in Fullerton, CA, is seeking a compassionate and strategic Care Management Manager (MSW or RN) to lead a dynamic team of Registered Nurses and Social Workers. This full-time, onsite position plays a key leadership role in ensuring patient-centered care, effective discharge planning, and seamless transitions across the care continuum.

Key Responsibilities:

• Oversee daily operations of the Care Management department

• Supervise and support RNs, MSWs, and support staff

• Facilitate effective discharge planning and care coordination

• Collaborate with interdisciplinary teams to drive optimal patient outcomes

• Ensure compliance with organizational and regulatory standards

• Lead performance evaluations, staff development, and improvement initiatives

• Support strategic planning and implementation of department goals

Qualifications:

Required:

• Bachelor’s degree in Nursing (BSN) or Master’s degree in Social Work (MSW)

• Minimum 3 years of clinical experience in acute or ambulatory care

• At least 1 year of recent leadership experience

• Strong knowledge of healthcare operations and project management

Preferred:

• Master’s degree in Nursing, Healthcare Administration, or related field

• Case Management Certification (CCM or ACM)

• 3+ years of healthcare management experience
