Director (RN) of Case Management

Overview:
Our client, a leading healthcare provider in Ottumwa, IA, is seeking a dedicated and experienced Director of Case Management (Registered Nurse) to lead their hospital-based case management program. This role is integral to driving high-quality patient outcomes, managing care coordination, and overseeing utilization and discharge planning. The Director will work closely with clinical staff and administration to ensure efficient, cost-effective care that aligns with best practices and regulatory requirements.

Key Responsibilities:

• Oversee the day-to-day operations of the Case Management Department, including care facilitation, utilization review, discharge planning, and quality management initiatives.

• Lead and develop a team of case managers and support staff.

• Ensure compliance with internal policies, clinical guidelines, and external regulatory requirements.

• Collaborate with interdisciplinary teams to ensure seamless patient transitions and optimal use of healthcare resources.

• Analyze performance metrics and implement improvements to enhance patient care and reduce length of stay.

• Report directly to hospital leadership on departmental performance, challenges, and strategic initiatives.

Minimum Qualifications:

Education:

• Graduate of an accredited nursing program (Diploma, Associate, or Bachelor’s degree).

• BSN strongly preferred.

Experience:

• Minimum of two (2) years in Case Management with expertise in utilization management, discharge planning, or related quality/cost management roles.

• At least two (2) years of hospital-based nursing experience.

• Leadership or management experience (2–3 years) preferred.

Licensure:

• Must hold an active RN license in the state of Iowa or a valid multistate license that permits practice in IA.
