Shift Details:

• Full-time, 8-hour variable shifts

• Schedule: 8:00 AM–4:30 PM or 4:30 PM–1:00 AM

• Rotating weekends required

Position Summary:

Our client, a leading healthcare system, is seeking Registered Nurse (RN) Care Managers to join their Care Management Team in Tarzana, CA. This is a pivotal role focused on case management, care coordination, and transitional care within a collaborative, patient-centered model.

The RN Care Manager will work closely with patients, families, physicians, social workers, payers, and the broader healthcare team to support timely discharge planning, reduce readmissions, and optimize health outcomes.

Key Responsibilities:

• Coordinate patient care across the continuum to ensure smooth transitions and appropriate resource use

• Support discharge planning, addressing clinical, psychosocial, and social determinant needs

• Advocate for patient-centered care, incorporating patient preferences and shared decision-making

• Collaborate with interdisciplinary teams to facilitate safe, high-quality care

• Monitor and manage utilization review and care efficiency metrics

Required Qualifications:

• Bachelor of Science in Nursing (BSN) or currently enrolled and committed to completion within 3 years of hire

• Active California RN license

• Minimum 2 years of nursing experience in acute, ambulatory, or post-acute healthcare settings

• Strong communication, coordination, and critical thinking skills
